What are their company SOPs, norms, habits, training, and requirements doing to incentivize behaviors that is not defaulting to that core principle?
This issue emerged in
bold relief (see what I did there?) after the AF447 crash report, and everybody freaked-out. Did it change anything? Maybe a bit of training to recognize a stall, but it is apparent the basic training problem was soon forgotten or blown-off.
