I was told by Avmed - no, unless clinically indicated. That can mean a number of different interpretations about trawling back through a Cl-2 applicants history whilst they were a Cl-1.
It depends. Consider the Class 1 extra requirements a "trawl" to see if any abnormality shows up. For Class 2, it is assumed the consequences of risk of inflight incapacitation is less than that for Class 1, so no "trawling" required. But obviously, if something shows up abnormal on the physical exam (or questionnaire), then the DAME/avmed might want extra testing specifically targetted at that abnormality.
