If there are altitude constraints, the normal Boeing technique us to set next constraint altitude on the MCP unless the cleared altitude is higher. Many pilots in normal ops use the alternate technique. If the autothrottle is inop I strongly suggest to use normal technique. The ’no big deal’ is not really acceptable as 1) you’re not alone and 2) you are human and get tired and distracted quicker than you think.

IIRC, the 737 actually has (low)speed protection in V/S mode and won't stall.
Not sure about high speed protection though.

The original question is made due to the lack of info about recomended modes for a Autothrottle inoperative take off. For Boeing 767 there’s is not a supplementary or bulletin regarding this. What I do is at 400’ HDG or LNAV, at acceleration altitude, ask for Climb Power in TMC, Speed at CMS or 250 kts, clean up flaps, and then flight level change or VNAV (maybe it will engage in VNAV SPEED), then Autopilot and climb with 10° pitch…..FCOM does not say too much about VNAV or its relationship with Autothrottle
