Boeing 787 water service access panel

I am attempting to identify events where the water service access panel on the 787 has come open and caused damage. I’m told there have been nine such events globally and that Boeing has issued a Service Bulletin aimed at mitigating the problem.
I understand Norwegian has had two events – in both cases the panel came off and was ingested. Virgin, two, but panel only damaged surrounding skins and, possibly, LanTam no details and may be wrong. I'd be interested if anyone can confirm and supply details of the possible LanTam event and identify the other four (?) events. I'd also welcome a copy of the Service Bulletin.

I know LOT had one which they got away without engine damage, about the time that Norwegian had one.
