Boeing 777 AFDS: VNAV, Climb Thrust & Single A/T Ops.
1. If VNAV is not armed prior to take off, will A/T mode change from HOLD to THR REF at 400 ft automatically?
2. Without VNAV when will thrust limit TO, TO1 etc. change to CLB, CLB1, CLB2?
3. If one, say, left A/T Servo motor fails, only right A/T is working, should one has to manually adjust and match the left thrust lever with the right lever all the time?

1) Incorrect. Automatic change does not occur. Hold mode remains engaged until until another thrust mode is (manually) selected or FLCH.
2) Incorrect. You have to change it manually either by selecting CLB/CON switch or FLCH.
