Dear all,

I need to find all forms that normally airlines have on board just in case it needs to be filled by the PIC after an event.
Here are some:

- bird strike
- lightning strike
- RVSM FL deviation
- TA/RA
- ATC events
- Severe icing/turbulence

Is there any website where I can get those?
If you are a freighter operator (no forms related to pax events) are there any other forms you may need to have on board?

we use one form as said above, Incident report form where the crew enter all relevant details. Some items on your list i.e Birdstrike, lighting strike will be covered by the tech log as maintenance actions will be required after such an event.

No form onboard anymore, any reports have to made after the flightduty in the crewroom using the online reporting tool. Future EFB III installations supposedly support online reporting on board.
