The vast majority of the time in UK procedures, where the hold axis is aligned with the inbound track of the final approach rather than the outbound track of the base/procedure turn, an alternative procedure of extending the outbound of the hold is noted, as in this case but only for the CEDAR hold, not the BHX hold. Not clear to me why it wouldn't be possible to extend the BHX hold outbound here as I can't see any relevant terrain -- it would be a long outbound of 9 miles, maybe there are limits because it's effectively DR?
