volcanic ash

Does your airline have a policy for operating with volcanic activity in the region? Ash on the ground ok? Zero tolerance for any amount of ash (say, reported ash and vis is 20 miles? 10 miles?).

Chapter/paragraph? I have seen some general information, I am interested in what individual carriers may have adopted as their own policy as well.
