Open cockpit door
I was on a flight operated by a certan German airline 
the other day, on an E190, and we pushed back, taxied and took off with the cockpit door open.
While it made for interesting view, and the 6am commuter passengers weren't particularly bothered, I suspect it wasn't intentional and perhaps the crew just forgot to close it. Once the seatbelt signs came off the stewardess went up to close it.
Has it happened to others?
