Having been involved in a number of startups myself including a commuter airline in VIE, Austria, a long haul B747 operator out of PIK and a charter carrier in ATH a will say that the most important factor is to survey the market and find a solution to your customers needs.
The 1st 2 carriers failed because the didn't survey the market or find a solution to their customers needs. The airlines in question were Eurosky Airlines GmbH & Highland Express Airways.
The success story was Viking Airlines who filled a need of both the market and customers alike.
