Two pilots or just one

Reading the posts about the Twin Otter crash it comes about that it had a single pilot. I'm not very plane savvy so does the Otter only have room for one pilot.
I thought that commercial (passenger) aircraft required two pilots as a fail-safe.
In which case what is the largest single pilot airframe and do any twin pilot airframes fly with only one guy at the stick routinely.

The Shorts SC7 Skyvan was single crew as are most aircraft with a MTOW of 5,700kgs or less, but dual controls normally fitted.
