Descent Rates/Altitude

Possibly a question for ATC or other forum, but my question is as follows.

From an aircrafts cruising altitude say FL350, how far from the arrival airport does ATC request for them to start their descent and to what altitude is this?

After first stage of descent, how far from the arrival airport is the aircraft told to approach at a set altitude?

So, at what distance from the destination airport would the aircraft be at in terms of altitude?

Hope this makes sense.

Edit to add:
Looking for an answer similar to e.g:

At 15 miles from destination airport aircraft should be approaching at 5,000 feet
At 10 miles ........... etc
