Given the quantity of debris in orbit, I am surprised that this sort of occurrence does not happen more frequently.

The problem is that near Earth orbit is not so big. A couple of decent sized satellites colliding in NEO could be devastating to the rest, not to mention the ISS and those astronauts/cosmonauts within.

The number of space debris is huge, and it increases every year, so I think we'll soon start to see such news every month.
