MAS blew 1 engine during take off

[Remark]
9M-MRI (cn 28416/155) About 10 seconds after liftoff from runway 19R something happens with the left engine. Alot of smoke and fragments of metal and other material falls down on the runway. At first the pilots don't get any indications in the cockpit and plan to go on to Kuala Lumpur. Then we decide to call Arlanda duty officer to make sure that they have noticed the pieces from the engine on the runway. Shortly after that the pilots requests fueldump and return to ARN safely.
Anyone know what went wrong in this incident ? Surprise that it can still take off with one engine. Real scary moment
Who is accountable for this ? Maintenance crew or manufacturer ?

This happened 3 Nov. Look in the Rumours archive for all the details. The engine cowling disintegrated. What you can see is mostly fibreglass and carbon fibre. The engine only suffered impact damage and continued to run. We changed the R Thrust Reverser, and sent it off.

Danger

er1cw This Boeing Aircraft as with all large multi engine jets is certified as a Performance A aircraft. you have a a decision speed on takeoff called V1 which is at or below the VR speed (speed to commence rotation for takeoff), once you have reached this speed the Aircraft can continue takeoff if it loses power on the most critical engine. Just to answer your Q about getting airborne on 1. Below V1 you abort takeoff,

In the course of testing and certification Boeing twinjets do more on one engine than 99% of the airline crews who fly them will ever experience or imagine.
