JCAB Medical Testing in the US

Are there any guys out there who know where I can get a JCAB Medical taken in the USA? I've google searched and the only one I come up with is the one in Alaska (a little too far). I checked out some of the previous posts on this but it hasn't helped any.

JFK Medical at JFK Airport does them, as does Dr. Frederic Costales of the Palo Alto Medical Clinic in the SF Bay Area. Hope this helps.
