Regarding the decision to return to LHR, I don't think you can discount that pilots weren't aware both engines were damaged. The incident happened on take-off, passenger reports wouldn't have made it...

Could be possible that crew were not aware BOTH sets of cowls were gone and treated it as single engine failure. Only the damage from one engine would have shown up in cockpit, doubt ATC saw both...
