EASA-land aircraft search forms

Has anyone been asked for an aircraft search form, especially at a French aerodrome? I got it from the OpsGroup forum but haven't heard about it from "official" sources, including the UKCAA.

Many thanks. Like you, I expect, I've seen lot of "correspondence" including an ICAO proposal letter (to change Annex 6 Part I and PANS-ATM) but when searching EASA's AMCs, have found nothing.
