Safety initiatives

Hi folks,

How many airlines out there have an " event trend analysis" program ie FDAP or FOQA ?
Are they also running these in parallel with LOSA ie Line Operational Safety Audit ?
Would be interested in hearing of your experiences with the above.

I seem to recall that Boeing introduced a scheme called PEAT - which I think stood for Procedural Event Analysis Tool - a few years back that airlines could use.
