Aviation Safety - TEM FOQA and LOSA

Can anyone please assist with difference between the above three exercises from a practical standpoint?
Do we really need LOSA when we have FOQA?

Do we really need LOSA when we have FOQA?
Yes.
FOQA (FDM) is based on data and shows what happened; what the aircraft did. The analysis is reactive, and subject to hindsight bias, but it can identify valuable trends and safety margins.
LOSA, with some reservations on how it is conducted, provides opportunity to understand why something has or might happen and provides insight of human behaviour. A well-considered output could identify proactive safety strategies and judge the effectiveness of human factors / CRM training.
