The lack of response shows the lack of real interest in safety and quality management systems in airlines. Any interest is rapidly damped out by the dullards who claim to be Flight Safety Officers, but are more often Slight Safety Officers.
Contrast with the ill-informed speculationfests that follow even the smallest incident here.

Jack Daniels;
You don't say where you are and you will find that SMS and Quality vary the world over so it is almost impossible to give you a straight answer.

He is a retired USAF Colonel with wide civil aviation experience, both schedulred and charter.
We also have old timers in the LHS, who actually pay attention to details.
Safety culture is top-notch, as a result.
