checklists

Has anybody ever seen a good template to make a checklist booklet? Im looking to make a normal and emergency procedures booklet that's about 5.5"X11" with tabs along the RH side for quick reference and wire bound along the left? Any help in finding a template would be greatly appreciated.

I don't have a template you're looking for, but if you go to http://www.caa.co.uk/docs/33/CAP676.PDF you can download the UK CAP676. It explains how to design Emergency and Abnormal Checklists including the Human Factor principles involved. I have used it in the past to make just A4 normal checklists before and found it a great resource. I hope this helps. Goodluck!

Jeppesen has those, but they are designed for plates. Works a treat. Comes with 15 or 30 (?) pockets as far as i can remember.
