"In accident investigation, you get what¡¯s called ¡®hindsight bias,¡¯" he warned. "We have to start with the way the world was, but then work hard to understand how the pilots perceived it¡ªwhat elements created the flawed situational awareness. Always ask yourself, ¡®What did they know, and when did they know it?¡¯

I picked this bit out from the article.It applies to so many cases.
