Police investigating four possible causes of An-12 crash near Lviv

Fuel starvation could be part of three of their four “causes”.
From this list the they seem to be looking more for the originator than the root cause.

Descended below the glide slope and collided with the ground in dense fog.
Previous flights were done by the same crew (34h work/17h rest).
