why don't all pilots switch on their floodlights/nacelle lights when crossing any runway or holding in position?
Not much help if you're approaching from behind, surely ?

Utterly agree with your line of reasoning.
How many of us can honestly say "I am immune from making a simple mistake." (Which I suspect this may well have been.) Or "I always make the time to double check"
Having done a couple of courses touching on this type of investigative/remedial approach, it is very clear that "blame and train" fails to address the problem. And it seems, from reading here and in the news, that it's not that uncommon.
This idea of "cleared to land" (Number three) is contradictory to me, and seems an obvious weak point, a link in the chain of contributory factors, already put in place.
If, for example, "failure to scan" was identified as a causative factor, one way this could be ameliorated a lot would be by having an extra pair of ears and eyes dedicated just to monitoring/helping/interacting with the aerodrome controller.
Oh, but they're short staffed. And busy. (Another link.)

This area is another application where ADS-B will be extremely helpful and possibly life saving. ADS-B will allow aircraft to see each other, and with incursion software on a surface map, give an appropriate warning of impending conflict.
Our airline is about to deploy an ADS-B based surface system on Class III EFB’s.

ADS-B also allows aircraft to see behind them, something that may have prevented the tragic runway collision at LAX (in this scenario, with ADS-B and a surface system, both aircraft would have seen each other and with warning software, alerted both aircraft.
IMHO, the ultimate solution will be an ADS-B based system that alerts both pilots and controllers of impending incursions. Current technology only alerts controllers, who then have to react/transmit to pilot/pilot react, etc., etc. Not the best scenario where seconds count as in this SFO situation.
An announcement by the FAA that in my opinion is a very big deal…..the announcement that Class II EFB’s will be fast tracked for approval for surface applications showing own ship. Until now, the only way to show “own ship” position was to go to a Class III unit. The price difference is substantial - $20,000 vs. $150,000 (US) per unit. It may take bodies/more bent metal, but I think we will eventually see a mandate for equipment like this.
