757 Uncontained Blade failure

Anyone have information on which airline had the 757 throw a blade right through the blade containment ring recently? No damage to fuselage. the 757 was an RR powered, E4B I believe.

ps. That was a fan blade

Not sure if it threw the whole blade or part of it. It made it through the Kevlar, but missed the airplane. Just curious as to the conditions and knowing the airline would help understand those conditions.
