Apparently this one.

A helicopter crashed near Fama, State of Minas Geais, around noon this Saturday (20). According to Military Police of Alfenas (MG), three people were in the aircraft, the pilot, Sgt PM of Fama, Marcos Antonio Alves and his wife, Livia Reis Carvalho. The helicopter pilot was rescued alive by fishermen who were at the lake. He has been taken to a hospital in Eloi Mendes (MG). The couple could not get out of the aircraft, which was submerged in the lake.

One newspaper reports:

"The pilot of the helicopter that crashed in the Lake of Furnas on the afternoon of Saturday (20), Fame (MG), was arrested and taken to the police station Varginha (MG) to testify. After the helicopter crash, police said, the driver fled the scene and only presented himself in Eloi Mendes (MG), a city that is 50 miles from where the accident happened, without trying to provide help to the victims"

Normally with these accident videos, although there may be some debate over the detail, you can normally get a general sense of what the pilot was trying to do and what went wrong. More importantly, (with the benefits of hindsight and infinite time), you can discuss what the pilot should have done to avoid\mitigate the accident, and hence at least learn something without having to make the mistake yourself.

In this case, however, I just can't work out what the pilot was trying to do or why. Was he drunk\drugged; showing off; had no experience on type (or even rotary)?

The flight path looks pretty similar to my first attempts to fly a jetranger in Microsoft Flight Simulator, which was at a time when I'd not been near a real helicopter and I was trying to use a computer keyboard to control the craft!

I'd suggest: Plenty of power (3 up) to showboat with a towering take-off and climbs without problem. At 12s - 13s he throws it around with large nose down attitude (still showboating), causing machine to start to sink. Already at high-ish collective, pulls more to arrest descent. Change of direction and possibly now downwind exacerbates situation leading to low-time instinctive response of pulling even more collective. Horn starts at 16s and sings all the way to impact as he tries to straighten the ship and level wings to get any ground effect he can from that glassy surface whilst working out how he’s going to get that RRPM back without dumping the collective at this late stage in the game. Very sad that he took two others to their watery grave in the process.

I wonder if he said in Mexican... "Watch This"

BOBAKAT 25th Sep 2014 14:34

"governor" OFF ?

Hughes500 25th Sep 2014 15:37

Looks like he had enough airspeed, not much wind ( mirror glass water ) so doubt he run out of power. I would guess he wanted to skim across water showboating and just got it wrong ! Wrong people picked up The Darwin award !

A flat calm and a rapid manoeuvre. He probably never saw the water before he hit it.

A graphic and disturbing demonstration of the combination of forward velocity and zero buoyancy.
