TRI(H) anybody know how?

Just rooting through all the new CAA forms on the website, trying to find how to apply for a TRI(H) under the new rules. I think I have found most of the info, except I need to submit an examiner's report form, and unless I am being really thick, there aint one for helicopters, only FW. Anybody done this yet under the new system, and know where I can get the form to put before my (foreign) examiner (who obviously doesn't have the UK forms)?
