The B206 can, and I am certain has lost its rotor through both mast bumping and TT strap failure.

Drag damper failure in a German CH53 mid '70s was pretty terminal too.

TT strap fails - blade departs - massive out of balance - transmission rips out - all happened in an instant.
Happened in Quebec once in the early 70s.
