AS for a 212/412, I have seen that three or four times over the last few decades, normally nothing wrong with the drive train but Power Turbine normaly gets over temped, did your pilot not report that before he continued with the flight.

No he didn't. It happened quite a few years ago in a part of SE Asia that, at the time, was generally considered comparable to the Wild West. In any case, it was a location where indigenous pilots never made mistakes.
