Student in R22 makes left-hand clearing pedal turn in hover, left skid touches ground
http://www.aaib.dft.gov.uk/sites/aai...ta__g_osms.cfm

R22 in high hover with tailwind loses control in right-hand turn prior to landing
http://www.aaib.dft.gov.uk/sites/aai...ta__g_rout.cfm

And, a re-issued report on a Bensen B8MR - inspector found subsequently that a larger rotor than spec'd in the permit had been fitted

I know, two R22s - but how many of them are there, compared with 206s and 120s?

Probably more a case of poor piloting skill due to lack of practice. IF as stated he was in a high hover then Vortex Ring shouldn't occur. My guess is that he was approaching downwind with a high ROD and tried to rectify the situation by peddling it round into wind. And therefore was in a VR condition.
