When I was at my old job, we got sent a report about it ( all AStar operators did) and it went into detail about hydraulic failure and the pilots incorrect procedure.
Now im not saying that is what happened, or that the pilot did the wrong action, all im saying is that is the information we were given and for company chief pilots to ensure all pilots how fly the AS350 were properly trained on the emergency procedures.
