part of his brain was splattered on the ground?? wow, and i thought the tabloids were bad for sensationalism... that was totally uncalled for... what do you expect to happen if hit even if at flight idle... my condolences to his family and to the people on board who now have a very negative memory of a positive machine...

I am surprised that this thread didn’t receive more comments at the time, and I resurrect it now to see it will attract more interest the second time around.

A ground accident in helicopters is perhaps the saddest of all and should provide pertinent lessons for everyone in the industry.

Such an accident involving a Dauphin is unusual. Is there anyone on this forum with something informative to add regarding this sad event?

I would have thought that there must be some contributory factor that has now entered the ’public domain’ in Malaysia that would be educational to our fraternity.

With a Dauphin, if you don't disengage the autopilot after landing the cyclic will motor forward. Not saying this is why it happened, but this wouldn't the first time.

you are right. Happened to Bond Helicopters on the Viking BD Platform in the Southern NS in about 1993. 365N, Autopilot left engaged, cyclic motored forward and bye bye HLO....

Not just 365; 355 cyclic will motor forward if autopilot not disconnected too. On r/r stops easy to forget to disconnect - or to re-engage basic modes before lifting, which concentrates the mind on a windy day!

You have to be careful sometimes with the 365N/N2 autopilot disengage button, as it is a sequential disengagement of the 2 lanes. I have had, on a couple of occasions, only one lane disengage when I haven't pushed the button hard enough. You get all the indications of a disengagement i.e. ALARM + AP on the CWP but one lane stays in. I always try to keep my hand on the cyclic as you can feel the stick move as the autopilot motors it forward.
