Zero Accidents - A shared Responsibility

Request: If anyone can help.

I am after a paper (70 page) Feb 09, 1995. Washington DC, Secretary Penn Document Number AD-A307192

This report is titled "Zero Accidents: A shared responsibility". A conference (FAA) was held in January 1995 with approx 1000 attendees in Washington DC, on flight safety - This report came out of that conference.

It is available on the CASI website - however, I am not in a position to pay for it.

Any other links on the general subject much appreciated.
