747-400 Evacuation

How many times has a full evacuation (i.e. use of slides etc.) been executed from a 747-400?

I recall a Qantas incident but can't recall any others!

How succesful were the evacuations?

I seem to remember a United 744 having a nosewheel failure and evacuating some where in the states. I remember seeing a picture.
