Change state of issue to UK

I want to change the state of issue of my JAR CPL to the UK. My current state of issue is fully JAR compliant. Looking at the UK CAA website gives a general idea of the process, but is not very clear.

What I have is: A JAR CPL with ATPL written, B737 and Canadair Regional Jet typerating + an unrestricted Class 1 medical.

Can anyone, eventually someone who has done it recently, shed a light on what forms I need to submit and the fees charged? Do I need to send my current license with the application? or what is the procedure?

HELP it's a paper jungle out there
