Form FCL 09
Does anybody know the CAA document number (SRG????) for the form FCL 09....they have informed me I need to complete one. The CAA web site lists about twenty docs however none are titled FCL 09 or have it in the description.
Any ideas ?

I have a restriction on my CPL (the one which says the License does not fully comlpy with JAR regs as I passed my ATPLs under the old CAA system), it means I can only fly UK registered aircraft.
I applied to remove the restriction (as I have more than 700 hours) and they said I must complete a FCL09.
