Help needed for uni work !!!!
Hello everybody,
I'm writting for uni a paper on aircrew fatigue and really would like to find some examples of accident or incidents of airliners that would have been caused /attributed to fatigue. I'm sure you guys ( and girls) will be able to point me in the right direction .

Short of any incidents where crews actually are (suspected to be) asleep at the controls (KAL007?) I would expect that fatigue would more likely be found under the heading 'contributory factors' than 'causal factors'.
It must be hard to prove an omission or error of commission was due entirely to fatigue - there are invariably many human factors which are part of the 'accident chain'. Fatigue can often be suspected as exascerbating other problems, such as poor monitoring, but rarely definitely identified as the root cause.
