747 Four Engine Flame-out Report

Looking for a web-mounted copy of the UKAAIB (or predecessor) report on Capt Eric Moody and crew's encounter with volcanic ash at 37,000ft south of Indonesia enroute Perth on 24 Jun 82 in BA009 (the classic four-engine flame-out).

Made a comprehensive Google Search and it would seem not to be on the Internet.

Probably a full report from the DGCA of Indonesia somewhere as well as an internal report by BA. Not sure what requirement there will be for the UK CAA to publish a report, despite it being a UK registered aircraft, but it is likely to be in their 'Foreign Reports' section!

We used to show a video, dealing with flight through volcanic ash, to new entry crews and as part of our refresher training programme. It included a very graphic account of Eric Moody's epic encounter together with an interview with the man himself. The shots of the windshield degradation and his description of turning final and standing on the pedals to see through the tiny remaining vision area put the whole episode into pretty stark perspective.... gulp, gulp!!
If you have problems with getting the report, the video might make a suitable alternative??
