EASA class 1 medical and PRK surgery without documentation?

Hey everyone,
I know it's a longshot, but living in North America trying to contact AMEs (in AeMC) has proven to be very difficult, so hopefully someone here can shed light on it, had previous experience or happens to be an AME.
Back in 2009 I did a PRK surgery (Eye refraction). All went well. Problem is, in 2022 the clinic who performed the surgery went bankrupt, the surgeon retired and no documentation is available on it everywhere.

My understanding I need to provide the numbers before and after the surgery. Problem is, as I mentioned - I have no access to that data.

Does anyone knows if it is possible to obtain the class 1 medical without PRK surgery record it?
My understanding is that I have to be evaluated by an Ophthalmologist REGARDLESS, so can that suffices? I do have an FAA and TCCA (Canada) Class 1 medical, both of them are aware of the surgery (Canada even requested, and were provided, back in the day, of that data).

(Note: I can't simply NOT provide the information. While can't be seen to the naked eyes, my own Ophthalmologist says any Ophthalmologist can see, either by imaging or looking through their instrument, that my eyes is cornea is artificially different then others, which 100% indicates surgery).
