Medical transfer from UK to other state

How does one go about getting a copy of earliest available medical application and examination report forms with regard to transferring a medical from UK to another EU state.

I did my medical in Gatwick many years ago. Close to where I did my ATPL exams.

Here is more information when going to the UK. My desired state just copied the requirements listed in the middle:

Found my answer...that's done by the medical assessor.
