CPL QXC - Form

For the CPL QXC is there a form that you need to obtain signatures on? I recall for my PPL QXC I was given a form which was signed by the tower at Carlisle and Durham Tees Valley.

Nope just get it signed off by the chief pilot, as long as its 2 full stop landings and 300nm+ and logged as separate sectors in your logbook.

You don't even need the signature from the chief pilot, it just needs to be in your logbook.
