ATPL Paperwork Requirements

Greetings All,

I'm a current A320 pilot for an airline in Europe and have lately reached the magical 1,500 hours and I feel a thaw coming on to the ATPL. I'm looking at doing a skills test privately in Germany for a UK issued JAR FCL license, but the CAA paperwork is driving me mad. As far as I'm aware I need the following forms from the CAA:

-SRG1119
-SRG1183
-SRG1187 the all important money form.

On the SRG1183 form though in section 8 there is a section to be certified by my FTO about integrated training. I did the integrated programme at Oxford a few years back, and have all their licenses, so shouldn't the CAA have all this on file already? Or do I need to fill everything out again, just to give the civil servants something to do? Is there anything else from the CAA that I will need for the ATPL sim?

I'm worse than useless at government paperwork, so any guidance on filling out the forms from someone who has already gone through this process would be much appreciated. Thanks in advance guys and girls.
