Unfreezing ATPL

Not sure whether this is the correct forum, but here goes anyway;

At my next sim check I am expecting to unfreeze my ATPL. I remember a large amount of stressful calculations to ensure I had enough hours for issue of CPL, so intend to make an early start this time.

Is this the correct form that I will need? Any others?

Looks like the right form, however where in the Lasors is a nice table stating all the hours requirements to be met for the issue and something about a ATPL skill test?!
I can't find it, but I'm told its there...

ok, found, it, section G, and then 1.240 and 1.295 of FCL for the skill test content. Now can I be really cunning and do my skill test in a C152?
