IAF without a name

Hi there, have a small question I hope someone here could help me with.




This is the Jeppesen chart for NDB VOR DME Rwy 35 at KAUHAVA, FINLAND.

At R-257 KAU/DME 14.0 theres a IAF that continues onto a 12 DME ARC to final. But this IAF dosn't have a name, unlike other IAFs I'w seen. How would I ask ATC for clearence to execute that spesific Instrument Approach using that nameless IAF and the DME ARC?

Is it coming off a STAR? If so, you'll be cleared by that star for that approach.

If not, request the 12 D arc from the west.
