Of course people fail. If you're not in current IF practice (and many haven't flown IFR since their last renewal) it is no walk in the park.
The main difference between the initial and renewal is that during a renewal, at the examiners discretion, you may repeat items whereas on your initial you get one shot at it.
Don't take your renewal lightly.
