UK CAA application form help

Regarding the UK CAA MCC course credit application form, I can’t find it on their website.

How do I find SGR\1164

“Applicants who meet the following criteria shall ensure that the complete and submit Application Form SGR\1164, together with the appropriate fee as per the Scheme of Charges,”

I have almost found what I am looking for, in the 'Scheme of Charges' pages I can find no mention of the MCC. It goes on with almost every thing else though. Has anyone got any hints on the charges???

Hi Connection Fee
It is listed in the Scheme of Charges under Exemptions. It is the last item in this list and the cost is £44.
