JAA to FAA Forms

I have a UK JAA PPL and am looking to go and do some flying in the states later this year.

Luckily, I have a few days in NY at the end of July and would like to sort my FAA conversion out then.

My question therefore is which forms do I need to fill out?? I have seen the URL's to them posted on here before but cannot find them at the moment.

Also, where in NY can I go to do a conversion. How much will it cost to get there (i.e. taxi) and how long does it take??

So many questions, but thanks in advance.
