VFR Flight Plan Forms - anywhere to download?

Anyone know where I can download a printable VFR Flight Planning form - the sort that include the headings:
FROM - TO - SAFETY ALTITUDE - PLAN ALTITUDE - TRK (T) - HDG (T) - VAR(W) - HDG (M) - G/S - DIST - LEG TIME etc
NOT the IFR plans available from AIS etc.
Any help would be most appreciated

I have already downloaded the CA48.pdf file - it's the wrong sort of flight plan and doesn't include columns for HDG T, HDG M etc. The kind of form I want is purely for planning a VFR flight via towns, VRPs etc (as you would find at flying schools) I've googled and googled and still not found one

Do you inquire about what FAA calls a flight plan, or the one that is also referred to as a navigation log?

I'm not sure if you want an empty flight plan or one with all the numbers filled in for you. If the latter, and you want to fly via things like towns, this one has the most comprehensive database of waypoints and will produce an A5 PLOG:

You're getting confused answers because of your heading.
"Flight Plan" has a specific meaning under ICAO and there is a standardised Flight Plan form available as you say from the AIS site. It's not an "IFR Flight Plan Form", it's equally valid for VFR.
What you seem to be after is commonly called a PLOG or Pilot's Log. The links above will lead you to some although it's cheap enough to buy a pad of them from your local friendly pilot shop.
Alternatively if you get Navbox ProPlan or QuickPlan it will do all the planning and calculations and print out the PLOG for you.
I have used ProPlan for a number of years and there seems to be general agreement that it's the best for VFR.

Thanks for your replies - apologies for the confusion - I was using the "Pilots Log" form from my local club which (as Mike Cross explained) rather confusingly calls it a "VFR Flight Plan" on the top of the sheet.
I don't want an online version which fills in the bits as it's for training purposes so has to be used only in conjunction with a wiz wheel/ruler/protractor and a half mil map.
