Airlines with Exterior View Cam or ATC Audio On IFE

Which airlines have an In Flight Entertainment System (seat back) display of feed from an exterior view camera, and which have audio feed of ATC?

United have audio feed on channel 9.

bmi have an exterior camera, don't know about anyone else though.

Emirates 777s have them, on take off and landing the nose camera view is also shown on the main screen.
I was fortunate to experience a landing at Kai Tak with this entertainment.

On recent flight Malaga to Munich - the Condor 757 had exterior cameras (forward facing) for departure and landing .

ANA has exterior cameras. Why don't more airlines (e.g., US-based) have them? Is it that big a deal to install and maintain? They are entertaining.
