Mid-Air @ Wedderburn

SY Radio 2GB reporting a mid - air at Wedderburn today at approx 0130Z.

One fatality reported, one aircraft crashed 'in bush' nearby, nil other details as yet.

Other aircraft said to have landed safely.

A sad event. No doubt, more to follow.

R.I.P.

Feels like of late we’ve well and truly exceeded the mid-air stats for the last century sadly.

Before the iPad bashing brigade turns up on the thread, the aircraft involved were part of a formation.

From their FB page...
Freedom Formation Display Team

The Freedom Formation Team can sadly confirm that there has been an accident involving some of our aircraft today in Sydney’s southwest.
We are working to fully support the relevant authorities as they take the lead in coordinating the emergency response. We are also supporting the team members and their families.
All questions should be directed to the authorities at this time.

And some industry stalwarts rant that an SMS is a tick and flick exercise because it costs them money, whilst accidents still happen…

I am imagining he refers to a Safety Management System - or at least a Risk assessment process that identifies and quantifies hazards and puts mitigating controls in place.

The main mitigating controls in Formation Flying are "Don't do it" and if that one can't be used, "don't screw it up" comes into effect.

So what is your suggestion with regards to improving aviation safety, opposed to a rant!!

A bit 'harsh'.....Hardly a 'rant'......I agree with no 2 advice from Mr HL....So, yr 'SMS' in formation work = keep your distance?
Same as mine when I used to teach it.

'SMS' these days usually equals much 'paper work' of dubious effect - hence my query..

Certainly not for a mature organisation, in most cases the complete opposite if they have the right people in place, together with a good just culture policy and a quick and easy digital means to report.

Safety Management System, Operations Manuals, Expositions
In the end, it all comes down to a human who may have a monetary lapse of concentration.

...so you would agree it would be difficult for 4 mates on a Sunday morning Jolly to justify a formal SMS?
As 601 says, all it takes in Formation flying is a momentary lapse of attention.

I think what comes out more often than not is that a collision occurs when doing something not in formation. That is changing position, re-joining, or something else, not related to actually flying in station. It gets especially dangerous when somebody does something that hasn't been discussed before hand. I'm not saying that has any bearing on why this accident occurred, however has been the case quite often, just like the recent jet accident. I've done a little bit of formation in light aircraft and it seemed fairly safe if you just stuck to the basics, after all you are barely moving relative to each other if in proper position.
