Mid-Air @ Wedderburn
SY Radio 2GB reporting a mid - air at Wedderburn today at approx 0130Z.
One fatality reported, one aircraft crashed 'in bush' nearby, nil other details as yet.
Other aircraft said to have landed safely.
A sad event. No doubt, more to follow.
R.I.P.

Freedom Formation Display Team
The Freedom Formation Team can sadly confirm that there has been an accident involving some of our aircraft today in Sydney’s southwest.
We are working to fully support the relevant authorities as they take the lead in coordinating the emergency response. We are also supporting the team members and their families.
All questions should be directed to the authorities at this time.

I am imagining he refers to a Safety Management System - or at least a Risk assessment process that identifies and quantifies hazards and puts mitigating controls in place.
The main mitigating controls in Formation Flying are "Don't do it" and if that one can't be used, "don't screw it up" comes into effect.

A bit 'harsh'.....Hardly a 'rant'......I agree with no 2 advice from Mr HL....So, yr 'SMS' in formation work = keep your distance?
Same as mine when I used to teach it.
'SMS' these days usually equals much 'paper work' of dubious effect - hence my query..

Certainly not for a mature organisation, in most cases the complete opposite if they have the right people in place, together with a good just culture policy and a quick and easy digital means to report.

As 601 says, all it takes in Formation flying is a momentary lapse of attention.
